1) Write the Safe model number and/or key number in the form*
* Safe models in grey color and over 10 yrs old please reach out to original manufacturer (First Alert)

2) Copy of your picture I.D. (Drivers license, passport, regular I.D.) is required.
3) Copy of your receipt, invoice, or packing slip. If you don't have a receipt, invoice or packing slip you need to have this form notarized.
